– Clinical Hypnotherapy Practice
Welcome to Clear Solutions Hypnosis, a clinical hypnotherapy practice operated by Helen Anderssen by accessing and using our services, you/the client agree to comply with these Terms and Conditions. Please read them carefully before proceeding.
Services Offered Clear Solutions Hypnosis provides clinical hypnotherapy services for various purposes. The services may be offered in-person, online or through other communication channels as deemed appropriate.
Client Eligibility Clients seeking services from Clear Solutions Hypnosis must be at least 18 years of age or have the consent of a parent or legal guardian. Individuals with certain medical conditions or psychological disorders may require a referral from a medical professional before starting clinical hypnotherapy sessions.
Client Responsibility You/the client are responsible for providing accurate and truthful information about their medical history, mental health, medications and other relevant details that may affect the clinical hypnotherapy process. You/the client are encouraged to actively participate and cooperate during the sessions. You/ the client also declare that you/the client will be free of drugs and alcohol when participating in this session.
Scope of Practice Helen Anderssen, as a Clinical Hypnotherapist, practices within the scope of her qualifications and adheres to all ethical and legal guidelines set forth by relevant professional associations and regulatory bodies. You/the client understand, Helen Anderssen is not a licensed physician, psychologist, or medical practitioner of any kind and that hypnosis should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, or doctor.
Results and Guarantees While hypnotherapy can be highly effective for many individuals, Clear Solutions Hypnosis and Helen Anderssen does not guarantee specific outcomes or results from the sessions. Results may vary depending on individual circumstances and commitment to the process.
Client Responsibility You/the client accepts and agrees that you/the client is 100% responsible for its progress and results from the Clinical Hypnotherapy Session. Clear Solutions Hypnosis and Helen Anderssen will help and guide you/ the client; however, participation is the one vital element to the Clinical Hypnotherapy Session’s success that relies solely on you/ the client. Clear Solutions Hypnosis and Helen Anderssen makes no representations, warranties, or guarantees verbally or in writing regarding you/the client’s performance. You / the client understands that because of the nature of the Clinical Hypnotherapy Session and extent, the results experienced by each client may significantly vary. By signing below, you/the client acknowledges that there is an inherent risk of loss of capital and there is no guarantee that you/the client will reach its goals as a result of participation in Clinical Hypnotherapy and Clear Solutions Hypnosis and Helen Anderssen’s comments about the outcome are expressions of opinion only. Clear Solutions Hypnosis and Helen Anderssen make no guarantee other than that the services offered in this Clinical Hypnotherapy Session shall be provided to you/the client in accordance with the terms of this Agreement.
Session Scheduling and Cancellations You/the client are required to schedule sessions in advance. We understand if you must cancel and reschedule appointments, and sometimes unavoidable events occur. We would also like you to understand that we are a small business with limited appointment times. We respectfully request that cancellations or rescheduling must be made with a minimum notice period of 24 hours. Failure to provide adequate notice may result in charges for the missed session.
Fees and Payment You/ the client are responsible for paying the fees associated with your chosen clinical hypnotherapy services. Payment methods and pricing details will be provided upon request.
Refunds You/the client are responsible for full payment of fees for the entire Clinical Hypnotherapy Session, regardless of whether you/the client completes the Clinical Hypnotherapy Session. To further clarify, no refunds will be issued.
Limitation of Liability Clear Solutions Hypnosis and Helen Anderssen are not liable for any damages or losses arising from the use of their services, except where mandated by law. You/the client agrees to indemnify and hold harmless Clear Solutions Hypnosis and Helen Anderssen, and their affiliates, from any and all claims, demands, losses, causes of action, damage, lawsuits, judgments, including legal fees and costs, arising out of, or relating to, you/the client’s participation or action(s) under this Agreement. You/the client agrees to defend against any and all claims, demands, causes of action, lawsuits, and/or judgments arising out of, or relating to, you/the client’s participation under this Agreement, unless expressly stated otherwise by Clear Solutions Hypnosis and Helen Anderssen, in writing.
Confidentiality Clear Solutions Hypnosis and Helen Anderssen maintains strict confidentiality of all client information and records in accordance with the applicable laws and professional guidelines. By signing this form, you/the client consent that Helen Anderssen may release information to a specific individual or agency if it has been determined that a child or elder is at risk of or is currently being abused; if you, as a client, are in imminent danger to yourself or others; or if a subpoena of records has been requested. You/the client also understand that, at any time, Helen Anderssen may discuss aspects of you/the client’s case with other colleagues keeping your full name and identity completely confidential always unless you have given permission otherwise. This Agreement is considered a mutual non-disclosure agreement. Both Parties agree not to disclose, reveal, or make use of any information learned by either party during discussions, Or otherwise, throughout the term of this clinical hypnotherapy Session. Confidential Information includes, but is not limited to, information disclosed in connection with this Agreement, and shall not include information rightfully obtained from a third party. Both Parties shall keep all Confidential Information strictly confidential by using a reasonable degree of care, but not less than the degree of care used by it in safeguarding its own confidential information. The obligation of the Parties hereunder to hold the information confidential does not apply to information that is subsequently acquired by either Party from a third party who has a bona fide right to make such information available without restriction. Both Parties agree that any and all Confidential Information learned as of the Effective Date shall survive the termination, revocation, or expiration of this Agreement.
Testimonials By agreeing to this form, I agree to allow the Clinical Hypnotherapist to use any testimonials provided, as directed by the you/the client.
Modifications of Terms Clear Solutions Hypnosis reserves the right to modify these Terms and Conditions at any time. You/the client will be informed of any changes, and continued use of the services implies acceptance of the updated terms.
Governing Law and Jurisdiction These Terms and Conditions shall be governed by and construed in accordance with the laws of Queensland, Australia. Any disputes arising from these terms shall be subject to the exclusive jurisdiction of the courts in Queensland.
By proceeding with our services, you/the client acknowledge that you/ the client has read, understood, and agree to this Terms and Conditions.
